Children Desiring God – Event Participation Request Form

Thank your for your interest in having a representative from CDG participate in your event.

1. Please provide contact information (name, telephone and email) for the event?

2. What are the scheduled dates of the event?

3. Who is sponsoring the event?

4. What is the purpose of the event?

5. Who (teachers, parents, pastors) will make up the audience?

6. Would you be willing to contact other churches in your area that may be interested in the event topics?

7. Would you be willing for CDG to contact current or potential customers who may be close in proximity to your event?

8. Would you be will for CDG to supply promotional materials (such as DVDs, catalogs, conference brochures, etc.) to event attendees at our expense?

9. How many sessions will a CDG representative participate in?

10. How long should they plan to speak for each session?

11. Will you provide CDG with audio (CD or MP3) of the CDG sessions for use at CDG's discretion?

12. Are you requesting a specific CDG representative for this event?


Location / Travel Information

1. Where is the location of the event?

2. What airport will we fly into?

3. What is the travel time from the airport to the event location?

4. When will you expect a CDG representative to arrive at your location?

5. When will the CDG representative be available to return to Minneapolis?

6. Will you be covering all of CDG’s travel expenses?

7. Will you consider covering the travel expenses for a spouse or ministry partner to accompany the CDG representative?

8. Will you provide transportation from and to the airport?
